  Never got to see the Doctor because they wouldn’t see me unless I left my credit card on file with them. It was bad enough having to leave a credit card on file but the paper they wanted me to sign would allow them to charge any service to my card that the Insurance wouldn’t cover. Since I would not sign this form with my information they declined to see me. Not very professional in my opinion considering all the issues with identity theft these days!
